Social Worker

Social Worker:
The Social Worker will provide intervention in order to assist in meeting the social, psychological, rehabilitative, protective, financial and discharge planning needs of patients requiring continuing care or adjustment to lifestyle changes following discharge.
Provides assessment and consultative services to ensure assistance for patients/families with personal and/or environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care in accordance with the philosophy, goals, and objectives of Howard County Medical Center.
Job Duties:
Assessment and Plan of Care:
Demonstrates ability to interpret problems of social, emotional, physical, and economic situations as they relate to the medical condition and/or hospitalization.
Demonstrates skill in conducting focused assessments with emphasis on patient coping abilities, friend/family support, at risk behaviors, patient compliance and abuse/neglect concerns.
Prioritize patient problems in terms of scope, consequences, and needed interventions.
Discern pertinent data regarding advance directives and take appropriate action as needed.
Identify teaching/learning needs and provide patient and family education.
Develop a discharge plan that achieves successful outcomes.
Develop plan of care, seeking patient/family endorsement of plan and/or resolve conflict/disagreement.
Foresee potential problem situations and intervene to offset adverse impact.
Consult with Manager as appropriate.
Document all the above in a timely and accurate manner.
Assessment and Intervention:
Demonstrate an ability to effectively deal with people of various disabilities and diverse backgrounds.
Demonstrate utilization of knowledge, adaptability, and skills related to meeting age specific needs of the patient population served.
Provide consultation/intervention regarding advanced care planning and social work needs for all patients referred to the department.
Demonstrate ability to manage sensitive issues such as, advance directives, POAs for health care, and guardianship.
Exercise skill in interviewing to elicit information.
Demonstrate ability to explore feelings and behaviors to assist patient in resolutions.
Maintain confidentiality and comply with professional ethics according to professional and departmental standards.
Interdisciplinary Practice:
Collaborate with other disciplines to define those patients who will need social work intervention for discharge planning to contribute to a reduction in length of stay, better resource utilization and ensure continuity of care.
Involve patient and/or family endorsement of plans and/or resolution of conflicts or disagreements.
Identify and coordinate discharge plans with appropriate community agencies, ensuring transference of appropriate information.
Supports team decisions and outcomes through actions and communications.
Attends and documents interdisciplinary rounds each weekday.
Reviews and updates all departmental policies as assigned every 2 years.
Receives referrals for Swing Bed admissions and coordinates with other members of the Swing Bed team to determine appropriate admissions into Swing Bed at HCMC.
Maintains documentation of Swing Bed patients.
Communication Skills:
Listens effectively, transmits information accurately and understandably, and actively seeks constructive feedback.
Uses simple examples, illustrations or analogies to explain concepts; writes clearly, logically and is concise.
Identifies critical issues to be communicated in complex situations.
Demonstrates small and large group communication skills to enhance and encourage open dialogue.
Demonstrates sensitivity and caring for the interests of others, using empathy to understand others perspectives, needs and concerns.
Utilization Review:
Coordinates utilization review for hospitalized patients to assure high-quality, cost-effective outcomes.
Performs and thoroughly documents insurance authorization for all hospital admissions in a timely manner.
Performs concurrent review of all hospitalized patients and ensures appropriate level of care according to defined criteria, including appropriate physician orders.
Provides communication to all health care providers and payers regarding clinical criteria for admission status and discharge planning.
Notify patient and care team about payor restrictions with coverage status.
Stays current on different payor authorization requirements including but not limited to Medicare, Medicaid, private insurance, VA, etc.
Communicates clinical updates with applicable payors as needed.
Issues and tracks denial/appeal letters as appropriate.
Coordinates physician reviews/appeals when appropriate.
Keeps medical staff informed on CMS changes/regulations regarding inpatient requirements.
Monitors and reports to the healthcare team facility quality indicators including but not limited to 30-day readmission, observation to inpatient stays, and inpatient stays less than 24 hours, etc.
Education/Qualifications/Skills/Abilities:
Licensed Social Worker (BSW) or Registered Nurse licensed in the state of Nebraska.
Minimum of 2 years or more of Social Work and/or RN experience required.
